For Stroke
When a loved one suffers from a stroke (What is Stroke?), it is a traumatic event. The loss of normal functionality after a stroke makes what once was a simple activity like walking, shopping, or simply getting out of bed not longer so simple.
Once the stroke is stablised, the road to recovery can begin. Recovering from a stroke to regain as much functionality as before depends on how well the brain is able to ‘re-learn’ using the unaffected parts of the brain to ‘take over’ the work of the affected parts.
This relearning is referred to as brain plasticity, also known as neuroplasticity. It suggests that the location of a given function in the brain (for example, speech, movement) can move to another area of the brain. This transfer can be activated by repetitive learning. This is done by sprouting of new synaptic connections and creating new pathways to unaffected parts of the brain.

Regaining full functionality after a stroke then is a matter on which part of the brain is stimulated, how it is stimulated and how frequently it is stimulated to relearn how to carry out the function.
This relearning depends on two key factors
- An effective Rehabilitation Plan on how to stimulate the brain.
- A high frequency of stimulation. The more frequently the brain is stimulated, the better and faster it is able to relearn.
Not enough stimulation or an ineffective rehabilitation plan may not be sufficient for the brain to relearn.
Core Concepts developed the Home Therapy Rehabilitation Model (HTRM) to take into consideration the desire for a loved ones’ optimal recovery in an affordable manner, without compromising on the quality of care. HTRM consists of three key components – Assessment, Planning and Supervision.
Moving from one key milestone on the road of recovery to the next requires the condition and status of the client to be correctly assessed. This is crucial to the correct planning of the rehabilitation therapy. Once a rehabilitation plan is formulated, the client undertake a series of therapy sessions under close supervision.
Once the next milestone is achieved, the client is reassessed and the plan revised if necessary before continuing with the supervised therapy sessions.
